This position will work collaboratively with Emergency Medical Services (EMS), Emergency Medical Technicians (EMTs), pharmacists, healthcare providers, behavioral health providers, social service agencies, and other community partners to identify and address medical, behavioral, social, and environmental factors contributing to frequent EMS utilization and barriers to appropriate healthcare. This position will maintain an assigned MICH caseload, initially anticipated to be approximately 10 clients as the program becomes operational, with caseload size adjusted based on program needs and client complexity.\r\n\r\nThis position will work alongside EMS personnel during community visits, with clinical and operational responsibilities divided according to the needs of the individual client and visit. EMS personnel may obtain vital signs, complete medication reconciliation, and perform other assigned EMS functions, while this position will perform comprehensive nursing assessments, health histories, physical assessments, care planning, education, and care coordination. During the initial implementation and development of the MICH Program, anticipated to continue for approximately the first year or until the program is fully operational, this position will also conduct Assessment, Evaluation and Review Services (AERS) evaluations as assigned. AERS responsibilities will include conducting in-home assessments, identifying health and functional needs, documenting findings, coordinating appropriate services and referrals, and participating in interdisciplinary care planning.
