EXAMPLES OF WORK PERFORMED:

 

Supervises, instructs or assists the residents in activities of daily living; such as bathing, dressing, eating

and personal hygiene.

 

Provides individualized care, active treatment and appropriate documentation under professional

direction.

 

Provides age-appropriate, ongoing interactions with residents during structured and unstructured

 

Participates in in-service training courses.

 

Participates in housekeeping functions in the resident living areas, including responsibility for their

personal effects, including laundry; mopping and dusting floors, furniture and fixtures; cleaning

appliances, cleaning bathrooms and replacing bathroom supplies, disposing of waste.

 

Plans and provides guidance in social, recreational, work and community activities.

 

Assists with day-to-day needs and feelings of the resident.  Documents and reports observations of

individual behavior.

 

Participates in staff meetings as an integral member of the resident interdisciplinary team.

 

Transports and/or accompanies residents to various activities in the community, to hospital or clinic

appointments or in any emergency.

 

Assists with planning menus, following prescribed diets, shopping for, preparing and serving meals

while on duty.

 

Ensures the safety of the residents is maintained at all times by providing a safe living environment.

 

Devises, implements and documents goal plans that emphasize skill development and have objectives

which are consistent with assessed needs and specified developmental services and programs, including

behavior Supports Plans and other required documentation.

 

Works cooperatively with all personnel involved in the active treatment of the residents.

Participates in training programs provided by all disciplines involved in the total resident program

and the follow through of such programs.

 

Requisitions clothing and inventories residents’ personal belongings.

 

Performs related work as required.

 

REQUIRED KNOWLEDGE, SKILLS ANDABILITIES:

 

Ability to conduct and implement functional assessments.

 

Ability to read and write.

 

Ability to prepare necessary reports and recording of resident program progress.

 

Ability to plan and supervise social, recreational and work for residents in an assigned living area.

 

Ability to assist with day-to-day needs and to record and report observations.

 

Ability to build and maintain relationships with residents.

 

Ability to keep and maintain records with accuracy and completeness.

 

Ability to lift and transfer residents (who typically exceed 50 pounds) utilizing recommendations as

listed on the Positioning/Mobility List.

 

Ability to follow procedures established for safe handling of kitchen equipment, food, and food preparation.

 

Ability to maintain effective communication with staff and residents, following individual communication

profiles.

 

Ability to implement proper practices and techniques of supervision of residents.

 

Ability to follow instructions and demonstrate flexibility and initiative with the job responsibilities.
