Responsibilities

→Manage a caseload of home health patients, completing skilled nursing visits, assessments, and care plan updates in the field
→Perform admissions, recertifications, and discharges, with accurate and timely OASIS documentation
→Coordinate with physicians, therapists, and the interdisciplinary team on changes in patient condition
→Educate patients and family caregivers on disease management, medications, and home safety
→Respond to changes in patient status and escalate appropriately
→Maintain documentation that meets Medicare, state, and agency compliance standards

Requirements

Active, unencumbered RN license in IL
Minimum of [1-2] years of RN experience, with home health experience required
