PHMC serves as both a direct service provider to individuals, families, and communities across the region and an intermediary agent, managing large-scale contracts, government and philanthropic partnerships, and multidisciplinary initiatives that require operational sophistication, strategic leadership, and deep mission alignment.

Family Nurse Practitioner will provide primary care services for adults, according to certification, in federal-qualified health center settings to diverse and underserved populations. Employee will report to the Medical Director.

Responsibilities

→ Understands and adheres to PHMC compliance standards as they appear in the PHMC Code of Conduct, Whistle Blowers and Conflict of Interest Policies.
→Keeps abreast of all pertinent federal, state and PHMC regulations, laws, and policies as they presently exist and as they change or are modified.
→Comply with HIPAA and Confidentiality Policies and Procedures as they apply to the job.
→Comply with Department of Public Health (DPH), The Joint Commission and other accreditation and regulatory agencies standards.
→Adhere to all PHMC Policies and Procedures
→Knowledge and adherence to Infection Control and Environment of Care Guidelines and Procedures.

Requirements

Ability to collect adequate client data, conduct a physical exam, interpret physical findings and manage or refer patients as conditions or protocols dictate.
Good verbal communication and writing skills.
Computer skills
Ability to communicate and work with variety of patient populations, some with complex needs
Ability to prioritize and organize workday to meet changing needs of program and clients.
Ability to work within a team framework and respond positively to team members concerns.
Ability to track work to completion and compile reports.

Three (3) years of nursing experience, at least two (2) of which shall have been as an Advanced Practice Nurse in a related specialty, preferred, but not required.
