The Office of MaineCare Services (OMS) is seeking a dedicated Social Services Program Specialist to support Maine residents with disabilities who are experiencing complex healthcare, financial, service coordination, and placement challenges. This position plays a critical role in helping individuals access appropriate services, navigate barriers to care, and achieve the safest and least restrictive living environments possible.

Location & Work Arrangement
Hybrid Position | Augusta, Maine
This position is based in Augusta, Maine and offers a hybrid work schedule, combining remote work with required in-office attendance, meetings, trainings, and business needs as determined by the department.
Key Responsibilities
Complex Case Coordination
Provide consultation, guidance, and support to Maine residents with disabilities and their care teams to address complex healthcare, financial, housing, and placement issues.
Facilitate multidisciplinary team meetings for adults who do not have a safe discharge plan or are experiencing other significant barriers to services.
Coordinate efforts among healthcare providers, community agencies, state programs, and support systems to identify and implement solutions.
Develop and support person-centered plans designed to promote safety, stability, and community integration.
Care Planning & Resource Navigation
Assist members and their support teams in accessing services, benefits, waivers, and community resources.
Assess eligibility and appropriateness for programs and services, including:
Brain Injury Waiver (BIW)
Developmental Disabilities Services (DD)
Adult Protective Services (APS)
Other state and community-based programs
Support members in obtaining services within the least restrictive setting possible.
Partner with providers and stakeholders to develop, monitor, and update individualized plans of care.
Documentation & Program Support
Maintain accurate and timely documentation in accordance with Complex Case Unit (CCU) protocols.
Prepare and distribute meeting summaries, recommendations, and intervention updates to member teams.
Track case activity and outcomes within departmental databases.
Ensure compliance with applicable policies, procedures, confidentiality requirements, and client rights standards.
Community & Provider Collaboration
Establish and maintain productive working relationships with:
Hospitals and healthcare systems
Community service organizations
Long-term care providers
Behavioral health providers
Case management agencies
State agencies and programs
Members and family supports
Educate stakeholders regarding available resources and service options.
Collaborate across systems to improve outcomes and resolve barriers to care.
Additional Duties
Perform other related duties and responsibilities as assigned.
Knowledge, Skills, and Abilities
Successful candidates will demonstrate:
Knowledge of healthcare systems, social services programs, and community-based service delivery models.
Knowledge of current standards of care and practices related to healthcare providers, facilities, and community agencies.
Knowledge of Department policies, procedures, programs, and guidelines.
Understanding of client rights, confidentiality requirements, and ethical practice standards.
Familiarity with community resources, disability services, housing resources, and public assistance programs.
Strong written and verbal communication skills.
Ability to assess complex situations, analyze case information, and make sound recommendations.
Strong critical thinking, problem-solving, and organizational skills.
Ability to build and maintain effective professional relationships while maintaining objectivity and professionalism.
Ability to work independently and collaboratively within multidisciplinary teams.
Minimum Qualifications
Required
Six (6) years of education and/or professional experience in:
Social services casework
Social services program development or delivery
Human services administration
Social services fiscal or budget analysis
Related human services experience
Current Licensed Social Worker (LSW) credential issued by the Maine State Board of Social Worker Licensure.
Preferred
Experience in one or more of the following areas:

Long-Term Services and Supports (LTSS)

Community-Based Services
Program Development
Social Work Practice
