As a Clinic Success Lead, you'll own the relationship with a portfolio of pediatric primary care clinics from onboarding through ongoing optimization. You'll be the bridge between our clinical model and the day-to-day realities of a busy practice - translating metrics into action, building trust with clinical champions, and ensuring that every clinic achieves meaningful outcomes for the patients they serve.
This is a high-impact, high-ownership role for someone who is equally comfortable digging into data as they are building relationships.
- Lead clinic onboarding through 30-60-90 day implementation milestones and beyond
- Serve as the primary point of contact for clinic leadership, PCPs, behavioral health care managers, and billing staff
- Support provider engagement and referral identification
- Ensure clinic invoices are paid on-time
- Maintain physical presence with clinics through regular programming of events including dinners, trainings/CEU, and other provider/admin engagement
- Identify clinics trending below performance targets and follow and develop tailored action plans
- Serve as a subject matter resource on collaborative care workflows, billing procedures and tracking reimbursements
- Navigate clinical partner’s EMR to upload and maintain accurate note sharing between behavioral health team and clinic partners
- Facilitate ad-hoc consultations between PCP teams and Triplemoon’s behavioral health team as needed
- Contribute to clinic success playbook development as the program scales
Requirements
~1 min read- 3–5 years of experience in customer success / account management / inside sales role, in a healthcare services model (familiarity with behavioral health, collaborative care, or value-based care models strongly preferred)
- Experience working in a primary care setting (Pediatric or family clinics) with exposure to variety of clinical EHRs (Office Practicum, eClinicalWorks, Athena, etc)
- Exceptional relationship-building skills across clinical and administrative audiences
- Experience preparing executive-facing reports or QBR presentations
- Strong project management and time management discipline - you keep multiple clinic timelines moving without dropping details
- Past experience with continuous improvement / operational improvement efforts in a clinical setting
- Tech-savvy / digital native required with experience using digital collaboration tools (Google apps, Asana, Notion)
- Data analytics experience with ability to identify trends, and translate insights into action
- Willingness to “dive-in” alongside belief that “no task is too small”
- Bachelors degree (clinical experience/credentials preferred)
- Ability to travel for on-site clinic visits (50% time on the road)
- Ability to thrive in a fast-paced startup environment and adapt to evolving priorities (past start-up experience preferred)
