Responsibilities

→Provide hands-on support with daily community integration and life skills activities.
→Drive individuals to various community locations, appointments, and activities — reliable and safe transportation is essential to this role.
→Identify and coordinate opportunities that promote social connection and community inclusion based on individual interests.
→Develop and maintain relationships with local organizations, businesses, and community partners to create inclusive experiences.
→Follow the Plan of Care developed by the Delegating Nurse and provide feedback on client progress.
→Prioritize and respond to client needs with professionalism and empathy.
→Ensure client safety and well-being in all settings.
→Maintain accurate documentation of care and community engagement activities.
→Listen actively to client preferences and advocate for their inclusion in community life.

Monday-Friday, 7:00 AM - 3:00 PM

Requirements

High School Diploma or College Degree.
Valid Driver’s License and reliable transportation required (this is a driving-intensive position).
First Aid and CPR Certification required
Certified Medication Technician (CMT) license preferred.
Strong interpersonal skills, with a passion for supporting people with intellectual and developmental disabilities.
Self-starter who is comfortable working independently in the field.
Flexibility, patience, and a strong commitment to community inclusion.
