Responsibilities:

Conduct comprehensive patient assessments and develop individualized care plans for home health clients
Coordinate and manage all aspects of patient care, including scheduling, referrals, and interdisciplinary team communication
Perform skilled nursing visits, monitor patient progress, and adjust care plans as clinically indicated
Educate patients and family members on diagnoses, medications, and self-care techniques
Collaborate with physicians, therapists, and other healthcare professionals to ensure continuity of care
Maintain accurate and timely clinical documentation in compliance with regulatory and agency standards
Ensure patient safety and advocate for optimal health outcomes throughout the care continuum

Requirements:

Active RN license in the state of Colorado (or compact license eligibility)
Previous home health, case management, or community health nursing experience preferred
Strong clinical assessment and critical thinking skills
Excellent communication, organization, and time management abilities
Proficiency with electronic health records (EHR) and clinical documentation
Valid driver's license, reliable transportation, and willingness to travel within Northern CO service area
Compassionate, patient-centered approach with a commitment to quality care
