Portage Physicians, PC is seeking a compassionate, dedicated nurse practitioner or physician assistant to join our primary care team. Responsibilities

· Provide comprehensive primary care within your scope of practice, including preventive services, chronic disease management, and evaluation of acute concerns.

· Conduct patient assessments, develop treatment plans, and order appropriate diagnostic testing.

· Prescribe medications and treatments within applicable licensure and practice requirements.

· Build lasting relationships with patients and support informed, shared decision-making.

· Collaborate with physicians, advanced practice providers, and care teams to coordinate treatment, referrals, and follow-up.

· Maintain timely, accurate clinical documentation.

· Participate in quality improvement and practice initiatives that strengthen patient care.

Requirements

· Graduate of an accredited nurse practitioner or physician assistant program.

· Current national certification as a nurse practitioner or physician assistant appropriate to the patient population served.

· Current Michigan licensure, or eligibility to obtain licensure before starting employment.

· Ability to meet applicable prescribing and credentialing requirements.

· Strong communication skills and a collaborative approach to care.

· Commitment to clinical excellence, professionalism, and patient service.
