Responsibilities

→Assist with the client’s participation in developing the Individualized Recovery Plan and help the client understand the diagnoses and plan components.
→Carry out duties as a member of the team, under the direction of the Program Manager and/or Coordinator.
→Must maintain client confidentiality in accordance with the RGHS policy.
→Provide support and encouragement to help clients socialize, interact in a milieu setting and facilitate social activities groups within the milieu.
→Assist consumers who have experienced social stigmatization with their sense of acceptance and affiliation.
→Provide practical help and mentoring, advocacy, coordination, side-by-side individualized support, problem solving and direct assistance to help clients obtain the necessities of daily living.
→Promote client self advocacy skills
→Participate in staff meetings and treatment review meetings with PROS team, MD and NP.
→Collaborate with the team to document consumer progress to maintain a permanent record of client activities according to the agency’s established methods and procedures.

Requirements

Valid NYS Drivers License required
Reliable transportation required
