What You’ll Do:

As a Physical Therapist at Thome PACE, you’ll help participants maintain and improve their mobility, strength, and functional independence through individualized therapy, exercise, gait training, and ambulation support. You’ll evaluate participants, develop and update Plans of Care, provide and oversee therapeutic interventions, and recommend adaptive equipment or other supports based on each participant’s needs. You’ll also play an active role on the Interdisciplinary Team, partnering with other care professionals to coordinate services, support transitions of care, and ensure changes in participant status are communicated effectively. In addition to direct participant care, you’ll help lead maintenance and group exercise programs, provide education to families and staff, and contribute to ongoing quality improvement efforts that enhance the overall physical therapy program at Thome PACE.
