Licensed Clinician Social Worker (IDD)

• →Serve as the lead mental health clinician for Community Options' Specialized Enhanced Supported Housing program
• →Review referrals and assist with the admissions process
• →Complete and submit initial and concurrent authorization requests in accordance with the level of care guidelines
• →Complete initial biopsychosocial assessments and individualized plans of care within 30 days of admission
• →Develop discharge plans that align with the principles of resiliency and transitional skills within thirty (30) days of admission
• →Provide clinical supervision and support within the group home ensuring all staff are trained and consistently implementing each individual's treatment plan

• Master’s degree in counseling, social work, psychology, or a related field and a license to 
  practice independently in TN, including LCSW, LMFT, LPC-MHSP
  
• Demonstrated ability to conduct diagnostic assessments, establish treatment plans, and 
  provide individual and group therapy
• Minimum of two years’ experience working with people who have IDD and mental health 
  and/or challenging behavioral needs
• Valid driver’s license with a satisfactory driving record
• Employment is contingent upon successful completion of checks of criminal background, 
  central registry, child abuse registry, and drug testing
