3M’s Services LLC is seeking an experienced and dedicated Qualified Professional (QP) to support our growing West Virginia operations. This position will provide oversight of services delivered through the Aged & Disabled Waiver (ADW) and Intellectual/Developmental Disabilities Waiver (IDDW) programs.

The Qualified Professional will be responsible for managing an assigned caseload, supervising Direct Support Professionals (DSPs), ensuring quality service delivery, and maintaining compliance with West Virginia Medicaid waiver requirements.

We are looking for a motivated professional who demonstrates strong leadership, accountability, and a commitment to person-centered care.

Responsibilities

→Caseload Management: Oversee assigned members, monitor service delivery, and ensure individualized needs and goals are being met.
→Staff Supervision: Provide direct supervision, guidance, coaching, and performance oversight to DSPs.
→Service Coordination: Collaborate with members, families, guardians, case managers, and other providers to ensure continuity of care.
→Compliance & Documentation: Ensure service documentation, progress notes, and required records are accurate, timely, and compliant with Medicaid requirements.
→Quality Assurance: Conduct routine monitoring, service reviews, and follow-ups to maintain quality standards.
→Staff Development: Support DSP onboarding, training, competency development, and adherence to agency policies.
→Risk Management: Identify and address concerns involving member safety, service delivery, staffing, and regulatory compliance.

Requirements

Bachelor's degree in a human services-related field, or non-human services related field required.

Must meet applicable West Virginia Medicaid waiver qualifications for assigned responsibilities.
Experience supporting individuals with intellectual developmental disabilities, older adults, or individuals with physical disabilities &/or behavioral needs.
Previous experience supervising direct care staff or managing a human services caseload preferred.
Knowledge of Medicaid waiver services, person-centered planning, and documentation requirements.
Strong organizational, communication, and leadership skills.
Ability to work independently, prioritize responsibilities, and maintain compliance standards.
Valid driver's license and ability to travel to service locations as required.
