Intensive Support Social Worker (Adult and Aging Services)

• Minimum 5 years of experience as a Social Worker III or in a supervisory role within NC DSS
• Strong knowledge of NC policy, practice standards, and quality improvement
• Ability to work independently while leveraging technology effectively
• Demonstrated adaptability, emotional intelligence, and change management skills
• Willingness to travel across North Carolina and maintain a flexible schedule

• →Adult Protective Services (APS)
• →Investigate reports of abuse, neglect, and exploitation
• →Assess need for protective services and develop safety plans
• →Collaborate with law enforcement, healthcare providers, and community partners

• →Guardianship & Legal Interventions
• →Initiate and manage guardianship proceedings
• →Prepare legal documentation and represent DSS in court

• →In-Home Aide & Support Services
• →Assess eligibility and level of care needs
• →Develop and monitor plans that support safe, independent living

• →Long-Term Care & Special Assistance
• →Evaluate placement needs for adult care homes or nursing facilities
• →Determine eligibility for Special Assistance and Medicaid services
• →Conduct ongoing reassessments

• →Case Management & Collaboration
• →Coordinate with multidisciplinary partners (mental health, housing, healthcare, public health)
• →Provide crisis intervention and long-term case planning
