Intensive Support Social Worker (Adult and Aging Services)

- Willingness to travel across North Carolina and maintain a flexible schedule
Responsibilities

- →Investigate reports of abuse, neglect, and exploitation
- →Assess need for protective services and develop safety plans
- →Collaborate with law enforcement, healthcare providers, and community partners
- →Guardianship & Legal Interventions
- →Initiate and manage guardianship proceedings
- →Prepare legal documentation and represent DSS in court
- →In-Home Aide & Support Services
- →Assess eligibility and level of care needs
- →Develop and monitor plans that support safe, independent living
- →Long-Term Care & Special Assistance
- →Evaluate placement needs for adult care homes or nursing facilities
- →Determine eligibility for Special Assistance and Medicaid services
- →Conduct ongoing reassessments
- →Case Management & Collaboration
- →Coordinate with multidisciplinary partners (mental health, housing, healthcare, public health)
- →Provide crisis intervention and long-term case planning
