About the Role

The Area Clinical Operations Manager leads field clinical operations within an assigned area, providing direct leadership and accountability for Nurse Practitioners, Registered Nurses, and Licensed Practical Nurses. You will help ensure patients receive timely, coordinated care while aligning clinician capacity, scheduling, quality, and performance with company expectations.

Reporting to the Director of Clinical Operations, you will work closely with Medical Leadership and cross-functional teams to resolve barriers, strengthen continuity of care, and support consistent execution across markets. This role has a dotted-line relationship with the Medical Director for clinical oversight and medical decision-making support.

Key Responsibilities

  • Lead, coach, and manage the performance of assigned field clinicians through regular one-on-one meetings, team huddles, performance reviews, and corrective coaching.
  • Translate Clinical Operations priorities, policies, and workflows into consistent area execution and reinforce scheduling, documentation, communication, supply, mileage, safety, and escalation processes.
  • Support clinician onboarding, field integration, competency completion, and readiness to practice independently in partnership with Education and Medical Leadership.
  • Monitor patient demand, staffing capacity, clinician schedules, and geographic coverage; address operational barriers and escalate recruiting or coverage needs to the Director of Clinical Operations.
  • Use area scorecards to identify trends, coach performance, and implement improvement plans; provide regular updates on patient risks, staffing, results, and action plans.
  • Monitor active patients for scheduling gaps and hold clinicians accountable for required follow-up and dressing-change visits, including scheduling future visits before leaving the patient’s home when possible.
  • Oversee rescheduling after cancellations, missed visits, hospitalizations, and visits that could not be started or completed; ensure hospitalized patients are appropriately placed on hold and followed after discharge to determine when care can safely resume.
  • Identify and address risks for gaps in care, disengagement, avoidable discharge, or failure to meet the ordered frequency of care.
  • Ensure required in-home initial visits are completed by Nurse Practitioners and in-person NP follow-up occurs at least every 30 days when applicable; ensure dressing changes and other visits are assigned to the appropriate clinical role.
  • Partner with Intake, Patient Care Coordination, Member Engagement, and other teams to strengthen handoffs and resolve barriers while retaining area accountability for clinical scheduling and continuity.
  • Monitor timely, complete documentation, including finalization of visit notes within established expectations, and reinforce medical-necessity documentation, procedure, supply, and patient communication workflows.
  • Serve as the primary operational escalation point for field clinicians; escalate changes in condition, patient safety concerns, scope-of-practice questions, and medical decision-making issues to the appropriate Medical Director.
  • Partner with Medical Leadership and Education on clinician competency, clinical performance concerns, and ongoing development.
  • Support audits, corrective action plans, payor requirements, and regulatory readiness; coordinate timely investigation and resolution of patient complaints and field service concerns.

Requirements

  • Bachelor’s degree in nursing, healthcare administration, business administration, or a related field, or equivalent relevant experience.
  • Active, unrestricted Registered Nurse license and a bachelor’s degree in nursing.
  • At least five years of experience in healthcare operations, clinical operations, field leadership, home-based care, or a related setting.
  • At least two years of direct people-management experience, preferably leading licensed clinicians.
  • Experience managing performance through operational and clinical quality metrics and coordinating staffing, scheduling, geographic coverage, and patient-service needs.
  • Working knowledge of HIPAA, healthcare documentation, patient safety, and escalation requirements.
  • Strong organizational and communication skills, with the ability to manage competing priorities and address performance concerns directly and supportively.
  • Ability to distinguish operational issues from concerns requiring Medical Leadership involvement and to build structure in a growing, multi-state organization.
  • Ability to travel within the assigned area and participate in field visits or ride-alongs as needed.
  • Experience in wound care, home health, home-based primary or specialty care, or another mobile clinical delivery model.
  • Experience leading Nurse Practitioners, Registered Nurses, and Licensed Practical Nurses.
  • Knowledge of Medicare, Medicare Advantage, commercial payors, and value-based care models.
  • Experience with clinician onboarding, competency validation, audits, corrective action, and regulatory readiness.
  • Experience supporting multi-state operations and rapidly growing markets.
  • Experience with BOP or a similar electronic health record, care-management, scheduling, or task-management platform.
  • Wound care certification or meaningful provider-level or nursing wound care experience.
