Social Worker/Counselor on Call-Hospice

We are seeking a compassionate, grounded, and skilled Social Worker to join our Roanoke team as a Counselor On-Call. • →Provide on-call psychosocial support to hospice patients, caregivers, and families by phone and in person.
• →Respond promptly during assigned on-call shifts and review reports, emails, and messages at shift start.
• →Provide crisis intervention, grief support, education, and counseling per the hospice plan of care.
• →Support admissions, changes in level of care, and end-of-life needs in collaboration with the after-hours care team.
• →Complete accurate, timely documentation and shift reports per agency standards.

• Master’s degree in social work (from a school of social work accredited by the Council of Social Work Education); LCSW or licensure eligible preferred;
• At least three years of social work or counseling experience; hospice or end-of-life care preferred.
• Strong skills in crisis intervention, grief support, case management, and critical thinking.
• Excellent communication, collaboration, and ability to work independently.
• Compassionate, emotionally mature, and mission-driven with a heart for end-of-life care.
