About Us:
At Human Services Administration Organization (HSAO), we are dedicated to providing comprehensive service coordination to individuals and families across Allegheny County. Our mission spans behavioral health, drug and alcohol services, justice-related programs, and community support. We believe in fostering recovery, resilience, and full inclusion in the community through collaborative, person-centered care.
Position Overview:
Adolescent D&A Case Management is a voluntary service designed to assist youth in Allegheny County who have community-based needs who have also been identified as having substance use related concerns. Adolescents are often referred by school-based entities, other service providers, or their parent/guardian. Once referred, individuals are screened and assessed to establish a psychosocial understanding of what their potential needs may be. As part of that comprehensive intake, individuals are also given the opportunity to develop a recovery plan with their case manager that outlines self-identified goals and the path by which they hope to accomplish them. From there, case managers can support individuals through a treatment process, through court proceedings, in their pursuit of acquiring non-treatment related support or even just in obtaining basic essential items. As part of this, Case Managers are tasked with muti-disciplinary cross system collaboration so that those who are being served are linked with all available resources in addition to having a solution focused experience with our services.
Key Responsibilities:

- Develop individualized service plans based on assessments and update them regularly to reflect progress and evolving needs.

- Facilitate interdisciplinary team meetings with stakeholders including families, school personnel, healthcare providers, probation officers when appropriate, and any other entities who may be a part of someone’s larger treatment team.
- Actively advocate for clients' access to needed resources and provide feedback to address systemic barriers.
- Collaborate with treatment providers and other community partners to develop discharge plans and post-release services.
- Serve as a liaison between clients, providers, and systems, ensuring smooth communication and care coordination.
- Maintain accurate records of interactions, including service plans, case notes, and assessments, in electronic health records.
- Ensure compliance with HIPAA and other confidentiality policies.
- Participate in program audits and quality reviews to ensure that services meet agency and regulatory standards.
- Participate in ongoing training to stay current with best practices in mental health, substance use, and criminal justice services.
