Psychosocial Coordinator

We’re searching for a Psychosocial Coordinator, someone who’s ready to be part of a growing team with amazing opportunities. In this position you will screen for the biopsychosocial risk of patients and/or families and to inform and/or develop risk intervention programs, which are directly related to patient care, education, and research. 

• →Administers the biopsychosocial risk screening tool with patients and/or families.
• →Takes primary responsibility for the identification of eligible patients.
• →Works with the team to administer the screening tool with patients and families.
• →Coordinates and documents the results of the screening tool to help inform patient care and/or research initiatives.
• →Develops and maintains effective working relationships and communicates relevant information, in a timely manner, to the team.
• →Develops and prioritizes goals which support the psychosocial needs of patients and/or families as a whole and are congruent with the strategic plan.
• →Participates in patient and/or family programs to assist in meeting the psychosocial needs of patients and/or families.
• →Contributes to the efficient functioning of the department. 

• Bachelor's Degree Social Work, Child Life, Psychology, Kinesiology or other healthcare field required
• Experience working with patients and families in a pediatric healthcare setting required
