As an Outreach Program Manager, you'll play a vital leadership role supporting adults with developmental disabilities who live independently in the community. This position combines direct impact, team leadership, and program oversight to help individuals achieve their goals and live their fullest lives.
Responsibilities
~1 min readAs the Outreach Program Manager, you will supervise and support a team of Outreach Case Workers and Direct Support staff while ensuring high-quality, person-centered services for individuals served. Key responsibilities include:
· Providing leadership, coaching, training, and supervision to Outreach staff
· Supporting participant assessments, service planning, and goal development
· Ensuring individuals receive appropriate medical, behavioral, and community-based supports
· Promoting skill development, independence, self-advocacy, and community inclusion
· Managing staff schedules and service delivery coverage
· Monitoring documentation, compliance, billing, and program quality standards
· Building collaborative partnerships with families, DDS, healthcare providers, and community resources
· Participating in after-hours on-call support as part of a leadership team rotation
· Responding to participant and staffing needs in a fast-paced, dynamic environment
We're seeking a compassionate and organized leader who is passionate about supporting individuals with developmental disabilities and helping staff succeed.
Requirements
~1 min read· Bachelor's degree in Human Services or a related field preferred
· Experience in developmental disabilities, behavioral health, human services, or a related field
· Prior supervisory or leadership experience preferred
· Strong communication, problem-solving, and organizational skills
· Ability to build relationships with participants, families, community partners, and staff
· Valid driver's license and reliable transportation
· Ability to successfully complete required background and driving record checks
· Experience supervising human services professionals
· Knowledge of person-centered planning and community-based supports
· A commitment to inclusion, dignity, and empowerment
· The ability to balance leadership, compliance, and direct service responsibilities
· A passion for helping people reach their full potential
