→Must be at least 18 years of age
→Must be extremely organized
→Possess excellent communication and interpersonal skills
→Must be professional, flexible, and have the ability to work independently
→Proficient in Microsoft Office; experience using an electronic health record (EHR) system is a plus.
→Must be available evenings and weekends

→Experience in community work and knowledge of community resources.
→Bilingual (Spanish speaking) is a plus.
→Requires reliable transportation, valid driver's license, and current auto insurance coverage.
